Responsibilities

• Coordinate and manage patient care plans across the continuum of care
• Collaborate with physicians, nursing staff, social workers, and families to ensure efficient discharge and follow-up planning
• Evaluate patient progress and adjust care plans to meet ongoing needs
• Advocate for patients to ensure access to appropriate resources and services
• Accurately document case management activities and patient outcomes in the EMR system

Requirements

• Current Colorado RN License (or Compact License)
• Minimum 2 years of recent Case Management or Utilization Review experience
• Active BLS certification
• Strong understanding of discharge planning, resource management, and healthcare regulations
• Excellent communication, critical thinking, and problem-solving skills
