Responsibilities
- Coordinate and manage patient care plans across the continuum of care
- Collaborate with physicians, nursing staff, social workers, and families to ensure efficient discharge and follow-up planning
- Evaluate patient progress and adjust care plans to meet ongoing needs
- Advocate for patients to ensure access to appropriate resources and services
- Accurately document case management activities and patient outcomes in the EMR system
Requirements
- Current Colorado RN License (or Compact License)
- Minimum 2 years of recent Case Management or Utilization Review experience
- Active BLS certification
- Strong understanding of discharge planning, resource management, and healthcare regulations
- Excellent communication, critical thinking, and problem-solving skills
