The VRS Social Work Assistant/Case Managers are responsible for providing services to WeCARE clients who are deemed to be employable with accommodation. The VRS Social Worker Assistant/Case Manager implemented the service plan with a focus on supporting clients towards employment while reducing barriers to employment. VRS Social Worker Assistant/Case Manager will also provide outreach to remind clients of pending appointments and attempt to re-engage clients if they have missed an appointment.

Additional duties as needed to ensure compliance with HRA's and UBA's expectations.

Skills in the following Service Plans:

Accessing services to address medical/health diagnoses. Accessing services to address behavioral health diagnoses.

Accessing services to address barriers to employment such as obtaining approved forms of government identification, interview clothing, childcare, Office of Child Support Enforcement, improving credit rating, etc.

Knowledge of various software systems including SEAMS

Associates must have excellent verbal and writing skills, strong interpersonal skills, and the ability to respond to requests from many people and departments. The ability to focus and have strong attention to detail is mandatory.

Knowledge of various software systems including SEAMS and EPIC.

Possess strong computer skills with knowledge of Microsoft Office applications.

Qualifications:

Bachelor’s degree in human services or related field required. Master’s degree program in human services or related field preferred
