OhioRISE Care Coordinator

Relevant Experience must be in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a human services field, providing community-based services to youth and their caregivers.
Hours: Full Time with flexible schedule to meet the needs of clients and families (remote work available after meeting 90 day probationary period)
As a Child Focus OhioRISE team member, you will serve as the point of contact for assigned OhioRISE youth. As such, you will be responsible for comprehensive care coordination for all youth and family service needs and working collaboratively across systems including but not limited to education, behavioral health, juvenile justice, developmental disabilities, hospital and family and children’s first councils in Eastern Hamilton, Clermont, Brown, Adams, Scioto and Lawrence Counties in Southwest Ohio.

Requirements:Degree Required: High School Diploma / GED PLUS 3 years of relevant experience
Degree Preferred: Bachelor’s Degree in Social Work, Psychology, Criminal Justice, Human Services PLUS 2 years of experience
Master’s Degree in Social Work, Counseling, Psychology, Criminal Justice Human Services PLUS 1 year of experience
