Core Medical-Perm is seeking a LPN / LVN Home Health for a job in Lakewood, Colorado.

Job Description & Requirements

Home Health LPN - Days

Territory:  Lakewood / Centennial

• One year of previous LPN experience required \t
• Requires knowledge in nursing and person-centered care concepts\t
• Ability to demonstrate competency in the provision of individual care of patients based on the patients' assessed age and developmental needs.\t
• HS GED\t
• Current state LPN license for applicable state of employment or current LPN license from a participating state in National Compact Licensure (Colorado only). \t
• Current Cardiopulmonary Resuscitation (CPR) required;\t
• Current Basic Life Support (BLS) certification from
  \tAmerican Heart Association, preferred\t
• Valid driver's license\t
• LPN IV certification preferred
